Behavioral Health Care Coordinator

Imagine Pediatrics is a pediatrician-led medical group providing virtual-first care for children with complex medical conditions. We seek a Behavioral Health Care Coordinator to support families through case management services.

Skills

Case management experience
Knowledge of mental health principles
Crisis intervention skills
Bilingual in Spanish
Proficiency in telehealth services
Written and oral communication skills
Organizational skills
Cultural competence

Responsibilities

Monitor high-risk pediatric patients and ensure appropriate follow-up and clinical management
Evaluate effectiveness of care plans and identify needs for transitions of care
Develop intervention plans addressing biopsychosocial needs of patients and caregivers
Provide case management support for mental health diagnoses
Support children and families in accessing quality care
Respond to crises to stabilize patients and family members
Provide training and education on mental health diagnosis
Consult with interdisciplinary teams to monitor patient progress
Conduct telehealth visits

Education

Master's degree in social work
License/certification (LMSW) preferred
