Nurse Care Coordinator

Imagine Pediatrics is seeking a passionate Nurse Care Coordinator to provide proactive care and triage for clinical issues, embed a family-centered care philosophy, and work with culturally diverse populations. The role involves collaborating with a multidisciplinary team to deliver continuous support to children with complex medical conditions.

Skills

Licensed RN in at least one state with eligibility to register for other state licensures
Minimum 1 year care coordination or case management experience
Familiarity with Medicaid regulations and services
Pediatrics experience strongly preferred
Virtual care experience a plus
Bilingual Spanish strongly desired

Responsibilities

Provide professional and friendly proactive care and triage for clinical issues
Establish rapport and a relationship with the patient and family to understand their needs and expectations
Establish a comprehensive plan of care in conjunction with physicians and the interdisciplinary team
Document all care management assessments and interventions
Refer to Social Worker or Behavioral Health for complex psychosocial and discharge planning issues

Education

Graduate with a bachelor’s in nursing from an accredited university preferred
