Intake Coordinator - Home Health

Our team is actively seeking an ambitious Home Health Intake Coordinator for a flourishing Home Health / Hospice company located in Houston. The Intake Coordinator will play a significant role in facilitating the intake referral process and ensuring seamless coordination of care between all involved. The primary responsibilities will include evaluating incoming referrals for appropriateness of admission. Additionally, the Intake Coordinator will verify benefits, collect patient demographics, and obtain all necessary support documentation. What You Can Expect From Us:

  • Competitive Salary: $45,000 to $55,000 (Depending on Experience)

Youll Be A Great Fit When You Have The Following Qualifications:

  • Proven ability to effectively handle and organize various types of orders, ensuring accuracy and adherence to protocols.
  • Prior experience in coordinating billing activities, including claim submission, payment processing, and resolving billing-related issues.
  • Familiarity and competence in utilizing the KINNSER or WELLSKY software platforms for efficient workflow management and documentation.
  • Experience and proficiency in utilizing the Availity platform for insurance eligibility verification and claims management.

The Key Responsibilities and Duties of This Position:

  • Assess the suitability of referrals for admission, considering the specific criteria and guidelines of the organization.
  • Coordinate with insurance providers to confirm patients coverage and ensure compliance with reimbursement requirements.
  • Gather accurate and up-to-date information regarding patients personal details, medical history, and contact information.
  • Collaborate with healthcare professionals to acquire all necessary documentation, including medical records, assessments, and treatment plans.
