Hiring Range: $26.89 – $33.61 per hour

SUDP or Trainee License Required

Kitsap Mental Health Services (KMHS) is looking for a dedicated Co-occurring Disorders (COD) Care Coordinator to join our multidisciplinary team. In this role, you will work with clients experiencing co-occurring mental health and substance use disorders, providing comprehensive care coordination, individual and group therapy, substance use assessments, and crisis intervention services. You will also collaborate with other community resources to ensure clients receive the care and support they need for a successful recovery.



Primary Responsibilities:

  • Provide care coordination, individual and group counseling, and crisis intervention services to clients experiencing co-occurring disorders.
  • Conduct substance use disorder assessments and determine level of care placements using the ASAM criteria.
  • Collaborate with the treatment team to develop and implement integrated treatment plans for clients.
  • Use evidence-based techniques such as motivational interviewing, harm reduction, and stage-wise treatment strategies to support client recovery.
  • Provide education and support to clients’ families and serve as an advocate for clients’ rights and preferences.
  • Coordinate with community agencies such as DSHS, court systems, and housing authorities to support clients in accessing necessary services.
  • Assist clients in finding and utilizing healthcare providers, including primary care physicians and dentists, while promoting health self-management skills.
  • Offer crisis intervention services and work with Designated Crisis Responders when needed.
  • Document client progress and treatment plans according to established policies and procedures.
  • Participate in daily team meetings and treatment planning reviews.
  • Monitor clients for Least Restrictive Alternatives (LRA) compliance and assist with LRA extensions and coordination with legal personnel.

 

Minimum Qualifications:

EDUCATION: Bachelor’s Degree in Psychology, social services, or behavioral health field.

EXPERIENCE: Entry level (no prior related work experience)

This position requires driving clients in a personal vehicle on behalf of the agency; therefore, the incumbent must successfully complete a motor vehicle history check, possess and maintain a current, valid driver's license in the state of Washington, and have reliable, insured transportation.

LICENSURE: Agency Affiliated Counselor Registration. Current licensure as a Substance Use Disorder Professional or Trainee

 

 

Preferred Qualifications:

EXPERIENCE: Experienced (minimum 2 years of job-related experience)

Experience in co-occurring disorders treatment, individual and group therapy, vocational services, or substance abuse treatment.

The capacity to work well with children and families.
