DescriptionThe Long Term Care RN assumes responsibility and accountability for delivering care to patients in Long Term Care and/or Assisted Living Facilities. The nurse demonstrates the ability to make clinical judgments efficiently with supervision, employing critical thinking and performance skills in coordinating patient care. Join a dedicated team that values professional practice and provides a supportive environment for your career growth.Skills• Current Registered Nurse License for the state of practice• Strong communication skills and proficiency in English• Computer proficiency• Understanding of patient confidentiality and HIPAA requirements• Ability to assess and interpret patient needs effectivelyResponsibilities• Utilize the nursing process to assess, plan, implement, and evaluate patient care in a Long Term Care/Assisted Living Facility• Perform focused interviews to identify specific patient needs• Assess signs and symptoms indicating changes in patient conditions• Document the patient’s plan of care and responses to interventions• Provide a safe and therapeutic environment for patients• Initiate and evaluate patient and/or family education• Monitor safety and effectiveness of the environment and equipment• Participate in activities to facilitate good interpersonal communications• Adhere to professional standards and policiesQualifications• Complies with all relevant professional standards of practice• Successful completion of new hire training applicable to job site• Current CPR certification if applicable• TB Questionnaire, PPD, or chest x-ray if applicable• Current Health certificate as per contract or state regulationEducation Requirements• Current Registered Nurse License for the state in which the nurse practicesEducation Requirements Credential Category• NursingExperience Requirements• Must meet all federal, state, and local requirements• At least 18 years of ageWhy work in Bronx, NYBronx offers a vibrant community filled with rich cultural experiences, diverse neighborhoods, and a strong sense of community.
