We are seeking a dedicated and experienced Home Health Physical Therapy Assistant (PTA) to join our team in Portland, OR. The ideal candidate will be responsible for providing high-quality rehabilitative services to our patients in their homes. This is a full-time position and flexible with your schedule!


Competitive compensation ranging from $79,000-$88,000/ year. (DOE)


Requirements:

- Graduate of an accredited physical therapy assistant program with either an Associate's degree in Science or Applied Science approved by the American Physical Therapy Association.

- Current certification or license as required by state law.

- Possesses and maintains CPR certification.

- A licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.


Responsibilities:

- Provide physical therapy services to patients in accordance with the established plan of care.

- Assist physical therapists in developing and implementing individualized treatment plans.

- Monitor patient progress and adjust treatment plans as necessary.

- Educate patients and their families about home exercise programs and activities of daily living.

- Maintain accurate and up-to-date patient records.

- Collaborate with other healthcare professionals to ensure comprehensive care for patients.
