About this Role 

Are you passionate about helping older adults stay active, regain confidence, and live their best lives? As a Physical Therapist in our PACE program, you’ll play a vital role on a collaborative Interdisciplinary Team (IDT), creating personalized care plans and delivering life-changing therapeutic care.


From leading fun and effective exercise programs to conducting home assessments and partnering with care facility staff, you’ll be the expert participants rely on to rediscover their mobility and independence. Your love for teaching will shine as you educate participants and families to keep progress going at home.

What does success look like as our Physical Therapist?
  • Collaborating with the IDT: Participate in interdisciplinary team (IDT) meetings to complete assessments, develop individualized care plans, and review service determination requests.
  • Assessing Participant Needs: Evaluate mobility levels, physical conditions, and functional abilities to create tailored therapy plans.
  • Providing Therapeutic Care: Deliver therapeutic exercises, gait training, and ambulation support to help participants achieve their goals.
  • Designing Therapy Programs: Develop engaging individual and group exercise programs focused on maintenance therapy and rehabilitation.
  • Delivering Evidence-Based Interventions: Provide physical therapy services aligned with physician orders and the participant's care plan, while documenting interventions accurately.
  • Identifying Priorities: Conduct thorough assessments to identify priority areas and recommend follow-ups when needed.
  • Referring for Additional Support: Recommend referrals for participants who may benefit from additional services or resources.
  • Empowering Participants and Families: Offer education to participants and their families, equipping them with the tools and knowledge needed for successful rehabilitation.
  • Managing Equipment: Assist with tracking, managing, and maintaining therapy equipment to ensure participant safety and program efficiency.
  • Collaborating with Care Providers: Work closely with staff in nursing homes, skilled nursing facilities, and assisted living settings to oversee rehabilitation needs for participants.
  • Supporting Care Plans: Provide approvals for care plans in nursing home settings on behalf of the IDT.
  • Bringing Care Home: Conduct home assessments and deliver in-home treatments to meet participants where they are most comfortable.

What does an ideal candidate look like?
  • A Heart for Service: Passion and mission orientation for serving high-risk seniors and frail older adults.
  • Specialized Expertise: Experience working with elderly or frail populations preferred, working with the Adult population required
  • Professional Preparation and Licensure: Graduation from a Physical Therapy program accredited by the American Physical Therapy Association. Must hold an active licensure in Physical Therapy (PT) and be in good standing with the appropriate state regulatory board.
  • Commitment to Compliance: Adherence to all federal, state, and HIPAA regulations.
  • Driving Requirements: Proof of a valid California driver’s license, personal transportation, a good driving record, and auto insurance as required by state law.
  • Essential Certifications: CPR and First Aid certification within 60 days of employment.
  • Physical Capability: Ability to lift, carry, or otherwise move up to 50 pounds.

  • Schedule is Monday - Friday 8am-5pm ONSITE
  • We’re excited to share that we plan to hire for this role in February/March, with our new site opening in July 2025! Onsite training will be provided at our Greater Los Angeles location to set you up for success.


$10,200 - $122,300 a year

Base pay is based on several factors including but not limited to education, relevant work and industry experience, certifications, and location of the role. Onsite roles include appropriate geographic adjustments, while remote roles are typically priced off national pay data.
