Title: Case Manager Reports to: Social Service Supervisor Summary: Perform specific Intake procedures to identify barriers that will impede the structure developed towards guiding clients in becoming self-sufficient. Provide case management and counseling to help them understand and overcome personal or social issues affecting their medical, housing, socio-economic, employment situations. Provide housing services to help residents understand and overcome personal or social issues affecting their housing and securing safe and affordable housing. Cooperate with social workers/social case managers to develop service plans to ensure permanent and stable housing access to community sources. Evaluate individuals' abilities, interests, and personality characteristics using tests, records, interviews, and professional sources to ensure appropriate services. In addition, staff is encouraged to be cross trained and have an understanding of job functions within their designated units.Responsibilities:Intake-• Review and access initial intake information and obtain additional information if necessary.• Create initial independent living plan.• Assist non-working clients to obtain Public Assistance• Complete bio-psycho socials within the first 30 days.Case Management and Counseling-• Maintain a caseload of approximately 22-27 clients• Provide counseling and referrals for special needs or general population clients• Meet weekly or bi-weekly to follow up on individualized Independent Living Plans• Develop savings plan and ensure continuous savings.• Produce passes for working clients• Conduct on-going individual and group counseling as well as crisis intervention.• Refer clients to internal and external resources and serve as an advocate when necessary.• Enforce facility rules and regulations to maintain compliance.• Participate in regular case conferencing and client review• Work collaboratively with housing and employment specialists to identify clients' readiness for employment and/or housing.Aftercare-• Provide case management and counseling periodically; one year beyond the clients' departure from shelter.General-• Attend staff meetings and serve on committees as required.• Attend professional meetings, educational conferences, and in-service training.• Maintain current and accurate documentation, progress notes, and data tools both physically and electronically.• Maintain confidentiality.• Maintain a professional appearance.• Time management and prioritization.• Maintain awareness of new trends and development in social services and related fields.• Interacting with other departmental staff• Perform general clerical duties• Develop contacts in the community for referral purposes.• Develops and maintains good communication with other programs and units within the organization to facilitate inter-agency referrals.Qualifications:• Baccalaureate degree in Social Work, Psychology, Sociology, or related field with 2+ years of direct social service experience.• Associate degree in Human Services, or related field with 4+ years of direct social service experience.• High School Diploma/General Equivalent Diploma with 8+ years of direct social service experience.• Knowledgeable of the plight of homelessness and special needs populations.• Knowledgeable of case work/case management methodologies.• Proven ability to work collaboratively well with diverse groups.• Proven ability to handle multiple tasks effectively under pressure.• Strong organizational skills, detail-oriented and efficient.• Maturity, integrity and sound judgement.• Job Type: Full-timePay: $35,000.00 - $44,000.00 per yearBenefits:• Dental insurance• Health insurance• Life insurance• Paid time off• Retirement plan• Vision insuranceSchedule:• 10 hour shift• Day shift• Evening shift• Monday to Friday• Morning shift• Overnight shift• Weekends as neededPeople with a criminal record are encouraged to applyWork Location: In person
