Value Based Care Medical Director - New Jersey (Hybrid)
Quartet Health is seeking a Medical Director to lead collaborative care teams in the Whole Health program, focusing on patients with serious mental illness. The role involves providing comprehensive, patient-centered care, and supervising a multidisciplinary team.
Skills
- Active Internal Medicine or Family Medicine Board Certification
- Experience within value-based care models
- Comfort and experience in working with electronic medical records
- Excellent communication skills
- Valid unrestricted driver's license
Responsibilities
- Provide oversight and clinical supervision for advanced practice clinicians
- Manage a small panel of high-risk, complex patients
- Conduct in-person, virtual, and home-based visits for patient care
- Facilitate multidisciplinary case rounds and develop patient-centered care plans
- Collaborate with external providers and community organizations for patient advocacy
- Ensure timely and appropriate documentation for patient care
- Participate in managerial responsibilities such as coaching and training clinicians
Education
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) Graduate
- Certified in Basic Life Support for Healthcare Providers
- Active medical license and DEA Controlled Substance Registration in New Jersey
