Strive Health is transforming kidney care with a high-touch care team dedicated to supporting patients with chronic kidney disease and end-stage renal disease. They are looking for a Care Coordinator to facilitate care and communication for their patients.

Skills

Excellent communication skills
Ability to manage confidential information
Proficiency in MS Office
Understanding of patient care procedures

Responsibilities

Perform outbound calls to patients and providers
Monitor patient hospitalizations and follow-ups
Coordinate with clinical resources and providers
Provide education materials to patients
Collaborate with clinical teams to manage patient care

Education

2+ years of relevant education or experience
Current BLS or CPR Certification
