Case Management Nurse

Case Manager- Contract to HireWork Hours: Day shift, 8:30 am - 5:00 pmJob Responsibilities:• Assess and create discharge plans to ensure patients receive quality medical care.• Implement individualized care strategies directed toward the most appropriate and least restrictive level of care.• Utilize both company and community-based resources to establish safe and effective case management plans for hospitalized members and effective discharge planning to prevent readmissions.• Collaborate with patients, families, and healthcare providers to develop individualized plans of care that encompass both acute care episodes and post-hospital discharge plans.• Communicate with all stakeholders to ensure quality coordinated care and timely services for all hospitalized members.• Advocate for patients and families to ensure their needs and choices are represented and supported by the healthcare team.• Perform work onsite at various acute and alternative level care facilities within the area.• Floating may be required to cover cases in another facility telephonically.• Depending on EMR access/availability, cross-covering in multiple facilities may be required based on the daily average census and PTO coverage.• Attend all Medical Management clinical rounds.Required Skills/Attributes:• Minimum of two years of varied clinical experience in a hospital setting with an emphasis on case management or utilization management.• Experience in a managed care organization is preferred.• Understanding of the healthcare delivery system, including post-hospital care such as SNF/LTAC/AIR criteria.• Must possess a valid Nevada driver’s license and maintain personal auto insurance coverage.• Must reside in the state of Nevada.Preferred Skills/Attributes:• Strong organizational and computer skills.• Strong written and oral communication skills.• Good problem-solving skills.• Ability to assess patient needs to coordinate appropriate levels of care.• Ability to collaborate with professionals, patients, and caregivers.• Independent case management knowledge and skills to formulate plans of care without direct supervision.• Ability to contribute to the organization’s efforts to eliminate racial and ethnic disparities in organizational performance and support cultural awareness.Licenses/Certifications:• Must possess an active Nevada RN license (preferred).• Case management experience.Dress Code: Business casualInterview Process: Video phone or Teams
