Growing together.This person shall oversee and be responsible for all contracted care coordination (CC) activities for the CHOICES program as defined in this Contract, or in TennCare policy or protocol. This is a field-based position requiring the CC to travel, assess, create plans of care, and authorize LTSS services within prescribed timeframes. This team is responsible for the direct and day to day management of our LTSS members in Davidson, Williamson and Wilson counties and surrounding areas.If you reside in Tennessee, you will have the flexibility to work remotely* as you take on some tough challenges.Primary Responsibilities:• Travel, assess, create plans of care• Authorize LTSS services within prescribed timeframes• The direct and day to day management of our LTSS members in Davidson County areaYou’ll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:• Undergraduate degree or equivalent experience• Current, unrestricted LBSW, LMSW or LCSW licensure or RN licensure• 1+ years of clinical or case management experience• 1+ years of experience in long-term care, home health, hospice, public health or assisted living• Reliable transportation to be able to conduct field visitsPreferred Qualifications:• Medicare/Medicaid experience• Experience with electronic charting• Background working in geriatric special needs• Background in behavioral health• All employees working remotely will be required to adhere to UnitedHealth Group’s Telecommuter PolicyAt UnitedHealth Group, our mission is to help people live healthier lives and make the health system work better for everyone.
