As a Per Diem MSW Social Worker at AdvisaCare Home Health and Hospice, your primary responsibilities include performing initial psychosocial assessments, developing and implementing goal-directed care plans, and documenting all patient interactions. To qualify as a Per Diem MSW Social Worker at AdvisaCare Home Health and Hospice, you must hold a master's degree in social work from a CSWE-accredited school. Additionally, having at least one year of experience in home health care and hospice experience is preferred. A valid driver’s license, car insurance, and reliable transportation are also essential for this role.
