Description

Salary:  $75,000 - $85,000 per year depending on experience 


Location: Albuquerque, NM


Full-Time, Monday-Friday

* Participates in on-call rotation

Summary:

On behalf of CNS Cares (“CNS” or “Company), This position is responsible to support the General Manager and Director of Nursing with a variety of administrative duties. The Clinical Supervisor will also provide direct patient care as needed.


 Employee must have regular attendance/punctuality, be able to work with others at all levels of the Company, have exceptional customer service, and be completely honest. Other assigned duties include:

  •  Develops an employee orientation program, implements it and conducts employee orientation for RNs, LPNs, CNAs and PCAs.
  •  Completes on site orientation and training of new hires.
  •  Follows up with new direct care staff up to 90 days post hire.
  •  Works with direct care staff closely to ensure employee retention.
  •  Evaluates clinical competencies upon hire and yearly.
  •  Assist to identify the needs of new clinical competencies and trains staff appropriately.
  •  Assists the sales team with HBEs per their request.
  •  Performs Initial Assessments or Admission interviews on all Hot Leads.
  •  Completes the Start of Care visit and all it entails for all new patients.
  •  Visits patients who are hospitalized, evaluates to ensure care level is appropriate to return to the home setting, assists with coordinating discharge with authorization team and hospital discharge care team.
  •  Completes in home comprehensive assessment within 48 hours of a patient being discharged from the hospital.
  •  Assist with covering vacations of corporate staff as requested by the General Manager.
  •  Provides direct patient care as described in state regulations for RN, in all cases involving nursing services as needed.
  •  Assists in the implementation of changes in the Plan of Care for the improvement of client care.
  •  Coordinates services for patients, as needed, to other agencies and health care providers as appropriate.
  •  Provides supervision of RNs, LPN’ s and Personal Care Assistant/Certified Nursing Assistant services as assigned.
  •  Maintains accurate time records.
  •  Submits required documentation in a timely manner.
  •  Be involved in patient care issues that are occurring in field in real time for advice, administrative involvement via communication note, phone calls, direct contact.
  •  Assists with employee annual reviews and competencies as assigned.
  •  Participates in on-call rotation.
  •  Other duties as assigned.

Requirements

Education/Training

  •  Bachelor’s degree in a related field, current Registered Nurse (RN) license to practice professional nursing in each state where patient care is supervised.
  • Current Basic Cardiac Life Support certification.
  •  Valid and current Registered Nurse license in the state of practice.
  •  Current CPR certification and IV therapy certification (if applicable)
  • Home health supervisory experience strongly preferred
