GENERAL OVERVIEW:

In collaboration with the Director of Care Management, facilitates and coordinates the provision of clinical social work/pastoral care services provided to patients and families.

ESSENTIAL RESPONSIBILITIES:

Provides direct supervision of professional social work practice/pastoral care services. Clinical coordination of social work services to patients and families. (20%)
Develops and maintains professional social work standards and requirements both in the departments and within the hospital committees. (10%)
Provides professional social work services to patients and families in the acute healthcare setting. (50%)
Supervises staff, evaluates work, and recommends and implements approved disciplinary action. (10%)
Ensure compliance with regulatory requirements. (10%)

QUALIFICATIONS:

Minimum

Master's Degree in Social Work from school of Social Work
LSW or LSW eligible in State of Pennsylvania
Five years’ experience in social work in an acute healthcare setting

Preferred

LCSW
Supervisory experience
