JOB SUMMARY:
The Health Coach is a Medical Assistant who, under supervision from the Clinic Site RN Manager or Clinic Units Manager, also serves as a member of the chronic care management team collaborating with patients and their providers to optimize the delivery of health care services to patients. The Health Coach provides medical assistance and interpretation, coordinates patient care with providers, and performs clerical tasks.
This is the entry-level trainee classification for the Health Coach position. Persons are recruited into this trainee position with the basic education, experience, and customer service skills required to perform all the duties of a Health Coach II, as described below. Under close supervision, incumbents undergo training, observation, testing, and evaluation in the full range of health coaching duties, as described below. Trainees are expected to complete the curriculum within one year, acquiring the knowledge, clinical competence, data gathering, and patient relations skills to fully and competently perform the duties of a Health Coach II. Promotion to the Health Coach II classification will be contingent upon successful completion of the training program as evidenced by the trainee’s performance evaluations and examinations.
