Provides nutrition education to patients undergoing bariatric surgery. Participates in overall program development and implementation, quality improvement and promotional activities of the department.• Assesses patient education needs and develops individualized education plan• Provides initial and follow-up consultations for diabetes patients and others requiring dietary modifications including patients planning bariatric surgery• Teaches Diabetes diet principles, disease process and treatment in group and individual sessions• Evaluates and develops teaching tools for use in education activities• Assists patients with setting goals to achieve optimal diabetes self management and monitors patient outcomes• Communicates with health team members (physicians, nurses, LCSW) regarding patient education and outcomes• Participates in departmental continuous quality improvement projects including patient chart reviewsQualificationsJob Requirements / Qualifications• BS Degree in Nutrition or Dietetics• Registered Dietitian• Master degree in related field preferred.Pay Range$22.89 - $33.20 /hour
