RN Care Manager - MA
Cityblock Health is seeking an experienced RN Care Manager to support integrated chronic disease and behavioral health care for our members.
Skills
- Active RN License
- Basic Life Support certification
- 3+ years of experience
- Technology proficiency for remote support
- Problem-solving skills
- Motivational Interviewing and Trauma Informed Care
- Adaptability and growth mindset
Responsibilities
- Manage a panel of rising and high intensity members
- Collaborate with members to create care plans
- Oversee progress and reassess needs
- Coordinate with providers and specialized resources
- Participate in case conferences and develop care plans with members
- Provide clinical support and medication reconciliation
- Ensure proper chart documentation and codes
- Support members in achieving their care plan goals
Education
- Graduate of an accredited school of nursing (R.N.)
