Home Health LVN Full-time or PRN

As a Home Health LVN at Pathfinder Home Health, your primary responsibilities include conducting skilled visits for patients, developing and revising care plans in collaboration with the RN Case Manager, and delivering quality patient care. Additionally, you will be observing and reporting any reactions to treatment or changes in patients’ conditions. Your role also involves instructing and supervising clients and their families in following the plan of treatment.

To qualify for the Home Health LVN position at Pathfinder Home Health, you must be a licensed vocational nurse with a valid LVN license. Experience in home health care is preferable, although we are committed to providing training for those who show promise and a willingness to learn.
