Phoenix Home Care and Hospice is on the lookout for a passionate and dedicated COTA Home Health - Lymphedema Certified specialist to join our wonderful team in the Overland Park, KS area, including Wyandotte and Leavenworth Counties. As a Certified Occupational Therapy Assistant, you will work under the supervision of our skilled Licensed Occupational Therapist to deliver exceptional care tailored to our clients' needs. You'll be responsible for providing essential occupational therapy services as outlined in each patient’s Plan of Care, ensuring compliance with federal, state, and agency regulations. Your role will involve direct interactions with patients, participating in case conferences to enhance care quality, and maintaining thorough documentation of the services you provide. With your COTA certification and ideally some experience in home care or rehabilitation, you’ll be a crucial part of a compassionate team that believes in fostering meaningful, innovative, and patient-centered care.
