What You'll Do:

Conduct in-home health risk assessments for members in your geographic area evaluate past medical history, review systems, reconcile medications, and perform vital signs and physical exams use evidence-based practice screening, including point-of-care testing as needed, Identify diagnoses for care management by Porter Care Guides actively manage medical treatment based on assessment findings, Formulate lists of current and past medical conditions using clinical knowledge and judgment.


What You'll Need:

Valid unencumbered Michigan NP license3 years of patient care experience required (experience in primary care/adult/geriatric, and EMR skills preferred) A strong sense of empathy, compassion, and friendliness, Exceptional communication skills, both written and oral, with the ability to assist others (patients and coworkers alike) with sincerity, respect, and compassion.
