Our LCSW will report directly to our Chief Medical Officer and play an integral role in the interdisciplinary care team. Remo LCSWs have deep empathy for families faced with dementia and the unique challenges that they face. They understand the toll that the disease can take, on everything from the emotional well being of the family to the practical burden of managing daily life. Remo LCSWs are committed to providing compassionate and comprehensive support to families, helping them navigate the complexities of dementia and find the resources that they need. With their specialized knowledge and expertise, Remo LCSWs offer guidance, understanding, and a sense of hope throughout their journey. Our LCSWs will assist families with engaging in advanced care planning, navigating benefit eligibility, and finding available community-based resources (such as options for food, housing , and transportation). Additionally, Remo understands the importance of creating a space for caregivers and those living with dementia to work through the difficult feelings that arise due to the diagnosis of a serious complex illness. To support this need, LCSWs will also offer psychotherapy services for those living with dementia and their caregivers.
What you’ll do
Provide compassionate and empathetic support, motivational interviewing, and education for dyads with the full support of a multidisciplinary team (neurologist, pharmacist, social worker, neuropalliative care doctor, etc.)
Support individuals living with dementia and their caregivers by assisting with and documenting advanced care planning, helping them navigate options for caregiving support at home and in facilities, and conservatorship
Help dyads access relevant support based on a needs assessment of the social determinants of health (e.g., Insurance, safety, social, financial needs, etc.). We see the social worker as the linchpin between Remo and local community-based organizations
Liaise w/ in-home partners (respite, safety eval partner) external to Remo on behalf of Dyads when a need arises
Work closely with Care Navigators and RNs to help navigate transitions of care (e.g., hospital discharge, skilled rehab stays, selecting a memory care facility, etc.)
Collaborate with the Chief Medical Officer and Lead Neurologist to contribute to regular educational conferences for clinical providers (e.g. clinical case conferences, neuroradiology rounds, guest lecturers)
Collaborate with the entire care team to deliver on our KPIs — e.g. quality targets, utilization metrics, patient/provider satisfaction, etc.
Participate in daily care team huddles and weekly care team conferences to discuss high risk patients
Support dyads and internal teammates through patient transitions of care, discharge planning, LTN placement, hospice placement, and death
Are personally motivated by our mission and dementia care innovation
Are invested in building relationships with community based organizations
Excel at active listening
Arere naturally empathetic and talented at building trusting relationships
Are willing to roll-up your sleeves, take ownership, multitask and dive into details
Have 5+ yrs experience particularly within geriatrics, internal medicine, palliative care, or dementia-related care
Have experience with advanced care planning and advanced directives
Are passionate about educating families and other clinical providers
Think systematically, breaking down complicated problems into manageable initiatives.
Have strong problem-solving skills and the ability to navigate challenges with creativity and flexibility
Have excellent written and verbal communication skills, with the ability to document progress notes and maintain accurate clinical records
Have experience navigating EHRs and encounter notes
Are comfortable adopting new technologies and learning new systems
Are comfortable with ambiguity
Lack ego and are driven by team outcome, not the individual outcome
Thrive in interdisciplinary teams (e.g. asking for help, collaborative problem solving, and mutual respect for all team members)
You are the ideal candidate if you:
5+ yrs experience particularly within geriatrics, internal medicine, palliative care, or dementia-related care
2+ yrs experience in digital health startup
Fluent in Spanish
