Clinical Care Navigator - After Hours

Spring Health is looking for independently licensed mental health professionals to join the Care Navigation team. The role involves providing clinical support, crisis assessment, and safety planning to members seeking help with mental health issues.

Responsibilities

Provide in-the-moment clinical support and psychoeducation to members
Assess for risks and provide crisis support and safety planning
Recommend plans of action based on clinical presentation
Coordinate connection to appropriate care services
Interact with multi-disciplinary teams to ensure a smooth member experience

Education

LCSW or similar licensure
Minimum of 2+ years of direct clinical care experience
