Serve as a key liaison between UMMS and the Post-Acute Care Collaborative, including Skilled Nursing Facilities and Home Health Agencies. Oversee program implementation to enhance patient care transitions across the continuum, ensuring quality, safety, and optimal outcomes. Lead day-to-day operations, communicate program goals, and manage project plans, challenges, and outcomes while engaging stakeholders and leadership. Work is performed under limited supervision. Direct report to the Vice President of Post-Acute Services.
Primary Responsibilities
• Effectively communicate program goals, expectations, and progress to business leaders, project managers, process owners, sponsors, and stakeholders.
• Cultivate and maintain collaborative relationships across the UMMS system to align program activities with organizational objectives.
• Serve as the primary liaison between UMMS and Post-Acute Care Collaborative Skilled Nursing Facilities and Home Health Agencies, ensuring consistent strategy implementation across member hospitals and partner organizations.
• Oversee data collection, analysis, and reporting to evaluate program performance and identify opportunities for improvement.
• Facilitate partnerships with post-acute care providers to enhance care practices and patient outcomes.
• Assist partners in defining and tracking key performance indicators for readmissions and length of stay. Utilize data-driven insights to support readmission reduction and length-of-stay goals.
• Develop and manage detailed project plans with clear objectives, milestones, and deliverables. Track project progress, identify variances, and implement corrective actions to ensure goals are met.
• Lead teams in problem-solving, promoting collaborative decision-making.
• Provide regular updates on program status, highlighting achievements, challenges, and recommended action plans.
• Perform other duties as assigned.
Education & Experience – Required
• Bachelor’s degree in Nursing.
• Minimum 10 years of healthcare experience, including 3 years in care coordination and/or post-acute program management.
Education & Experience - Preferred
• Master’s degree in Nursing, Public Health, Healthcare Administration, or Business Administration.
Knowledge, Skills, & Abilities
• Working knowledge of clinical and operational aspects of post-acute care, including payment
models and assessment tools.
• Working knowledge of risk-sharing payment models and business plan development.
• Advanced proficiency in data analysis tools and healthcare information systems.
• Working knowledge of growth and development principles across life spans to address age-specific patient.
• Proven ability to successfully motivate and inspire team members to high performance levels.
• Effective leadership skills, including situational awareness, conflict resolution, establishing alignment and clear direction, and driving continuous improvement.
• Expertise in developing, managing, and evaluating budgets.
• Proven expertise in prioritizing tasks and auditing work quality.
• Advanced skill developing and maintaining collaborative working relationships with all levels of leadership, providers and staff.
