Restorative Program Coordinator (RN or LPN) 

Salary:  RN $40-43/hr  LPN $35-37

Status:  Days full time, 40 hours per week

Schedule:  Days, with Manager on Duty (MOD) weekend rotations

Southview Manor (formerly Southview Care) is an inner-city skilled nursing facility (SNF) with 5 floors and an average census of 160+.  We have and multiple levels of care, including long term care and a large behavioral health (many formerly indigent) population.

  • The average age of our patients is 35-55 
  • Many are able to age in place, and about 25% return to the community.
  • We have several programs encouraging engagement, but the most popular spot in our center is the game room. 
  • We love new grads and work with students from multiple schools/universities in the area.

Job Description

What you will do in this role: 

The Restorative Program Coordinator (RN or LPN) is responsible for coordinating restorative care for our center.  You will assist residents in attaining or maintaining the highest level of physical functioning possible.  You will work closely with our therapy team to help patients reach their clinical goals and prevent declines.

Qualifications

What we need from you:

  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in IL
  • 2+ years’ experience in a SNF setting as a Restorative Nurse
  • The restorative/rehabilitation certification is ideal, but not required
  • Electronic medical records experience is helpful (Point Click Care or PCC is ideal)

These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.
