The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.
An Outreach Case Manager provides coordinated stabilization and time-limited case management support to clients in housing following exits from unsheltered homelessness or other transitional shelter sites. The Outreach Case Manager operates in a close team model within a larger collaborative of fellow case managers, outreach workers, outreach behavioral health navigators, street medicine nurses and providers, housing navigators, and other service providers from CCH and partner agencies dedicated to rapid exits from homelessness into housing and intentional follow-up care for at least the first year of housing. Services provided will include but are not limited to: relationship building and trauma-informed engagement, crises intervention, provision of basic needs ( food, clothing, household items, etc.), medical and behavioral health care, transportation, client advocacy and linkage to services and supports necessary for stabilization in housing following a time-limited housing subsidy. This position collaborates closely with outreach and case management teams internally within the Denver Street Outreach Collaborative and externally with Denver’s HOST (Department of Housing Stability), and also with non-congregate shelter providers, housing programs, and other City or contracted entities as needed to best support retention in the program, remove barriers to stabilization, and pursue a variety of housing options beyond the initial period of intervention.
NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.
