Care Management Coordinator

Overview:In ensuring that patients receive the highest standard of care, the Care Management Coordinator (CMC) is tasked with finding appropriate care solutions that meet their personalized needs. The CMC is an advocate for available resources in the community, while also exploring the entire continuum of care to offer the best care outcomes to their patients. This position regularly interacts with healthcare staff of all disciplines.Responsibilities:Assists patients and families in navigating through the healthcare system provided by the organization, guiding them towards the most appropriate course of treatment.The well-being of patients is largely dependent on the expertise and attentiveness of healthcare providers.Serves as an intermediary connecting patients to healthcare providers and community resources to enhance their overall wellbeing.To optimally connect patients with appropriate treatment choices, it is essential to understand the differences between Medicare Advantage Plans, Physician Provider Networks, and Employer Plans.Modifies care strategies for individuals to support them in accomplishing their short and long-range health aims, displaying them therapy alternatives to facilitate their progress.Executes any other tasks given.Qualifications:EDUCATION:Required:• Associate's Degree in NursingPreferred:• Bachelor's Degree of NursingEXPERIENCE:Required:• Five years nursing and/or home related care experienceLICENSURE/CERTIFICATION/REGISTRY/LISTING:REQUIREDRequired:BLS (CPR)-American Red Cross or AHA Healthcare Provider, RN Licensure
