Moreover, the incumbent will assist in enhancing patient and provider involvement and facilitating care coordination initiatives for Medicare Advantage and Commercial ACO patients linked to Triad Healthcare Network.This role involves regular travel to various Cone Health Clinics within the designated area.Responsibilities:Coordinates patient care by reaching out to patients and providers about HEDIS measures, as well as reviewing patient records and ensuring accurate documentation.Recognizes patterns within office settings or team environments and conveys conclusions clearly to supervisors, healthcare educators, or quality performance departments.Assists in conducting monthly assessments of physician involvement records and additional quality reports to determine chart review priorities or support team objectives.Encourages a constructive work atmosphere within a team that may face pressure from changing and rapidly growing business requirements, strict time constraints, and substantial work volumes.Assistance employs different Electronic Medical Record (EMR) Systems in a restricted view to pinpoint precise additional information.Carries out additional tasks as required.Qualifications:EDUCATION:Required:• High School Diploma or equivalentEXPERIENCE:Required: 1 yearLICENSURE/CERTIFICATION/REGISTRY/LISTING:REQUIRED
