About myPlace Health

myPlace Health was founded in 2021 by mission-aligned healthcare leaders and organizations that are committed to drastically improving health outcomes, quality and experience for vulnerable older adults and frail seniors. We specialize in providing value-based, comprehensive care and coverage for older adults with significant needs so they can thrive in the homes they love and in the communities they cherish.

Our mission is simple: to enable older adults to live the independent lives they deserve. We pursue this mission through our myPlace PACE (Program of All-Inclusive Care for the Elderly) model, which provides seamless primary care, integrated health plan coverage, personalized social engagement, and customized services delivered in the participant’s preferred place.

myPlace Health is building a mission-driven team that shares our passion for redefining the way older adults experience care as they “age in place” in the community. This is a unique opportunity to take on one of our country’s most challenging healthcare problems and join a fast-growing, dynamic team as we prepare to scale our mission to serve more markets.

About this role

Are you passionate about food, nutrition, and making a difference in the lives of older adults? Join our PACE program and become part of a compassionate, mission-driven team dedicated to improving health and spreading joy.

As a Registered Dietitian, you’ll create personalized nutrition plans, oversee delicious, tailored meals, and lead fun wellness activities like cooking demonstrations—all while building meaningful connections with participants and teammates. Whether you're collaborating with our interdisciplinary team or mentoring staff, your work will leave a lasting impact.

If you're ready to bring your expertise, warmth, and creativity to a supportive and rewarding environment, we’d love to meet you. Let’s change lives together—one meal, one smile, and one connection at a time!

  • Train and Empower Staff: Provide regular training to day center staff on food service best practices, fostering a culture of safety, quality, and participant care.
  • Ensure Program Compliance: Review menus and other program components to meet the requirements of the Child and Adult Care Food Program (CACFP), ensuring participants’ nutritional needs are met with care.
  • Support at Home: Coordinate and document the delivery of oral nutrition supplements and home-delivered meals, ensuring participants feel supported both at the center and in their homes.
  • Document with Care: Maintain accurate and timely documentation of participant interactions and care plans in the EMR, reflecting the quality and thoughtfulness of the care provided.
