What is the purpose of this role?

The Clinical Practice & Family Advocate Lead plays a critical role in ensuring both effective practice operations and high- quality patient care. You provide on-the-ground training in the market, and step in to deliver in-person and telephonic care as needed, and support smooth practice operations through schedule management, vaccine program oversight, and administrative leadership. The Clinical Practice Lead helps ensure a seamless experience for both patients and providers, reinforcing Nest's commitment to delivering high-quality, accessible care in the home.

What will you do?

- Staff Training: Learn and maintain best practice understanding of Field and Office Family Advocate roles and become a power user of core tools (Athena EMR, Salesforce, SharePoint, and others). Partner with Learning & Development team to facilitate training programs for Nest Family Advocates (FAs)

- Train new staff in the office and field roles. Retrain other staff as directed by the Health Services Director (HSD)

- Perform skills checkoffs with all new hires

- Record effectiveness of skills and other ongoing trainings required for FAs

- Direct Family Support: Act as a back-up Family Advocate, providing in-home care and telephonic support to Nest families, which may include but not be limited to the following:

- Collaborate with the interdisciplinary team to proactively contact families and ensure they are connected to the providers, resources, support they need to meet their Lead Family health goals

- Assist in patient registration, checking and confirming eligibility, establishing administrative workflows, and prior authorization approvals

- Perform tasks under the direction of the licensed provider including taking vital signs, updating patient history, collecting lab specimens, and assisting with exams

- Administer oral, topical, and injectable medications and/or vaccinations

- Interview patients and record medical history including standardized screening questionnaires

- Assist providers in procedures using aseptic techniques

- Complete accurate EMR documentation of patient information and services provided per the requirements of Nest Health

- Provide information and assistance through advocacy, navigation support, and education to Nest families on availability and eligibility of health and social care services

- Clinical Operations

- At the guidance of market triad, construct and manage schedules in Athena and Salesforce to reflect appropriate staffing and facilitate smooth clinical operations

- Act as clinical lead on the Vaccines for Children (VFC) process and maintenance. Monitor vaccine par and coordinate ordering with the office manager

- Fulfill clerical responsibilities which may include sending/receiving/scanning medical records, obtaining diagnostic reports, hospital notes & referral information, completing forms/requisitions, managing the charting process & sending specimens to labs

- Actively participate in all compliance-related matters including, but not limited to, equipment, laboratory tests, medications, and vaccine inventory

- Administrative Operations:

- Maintain productive physical operations and space, including office supplies, mail, and invoice processing, and vendor management (e.g. for IT and field-based security)

- Manage mileage and payroll tracking and audits

- Assist with meeting organization, events, performance reporting and projects as needed

- Team Culture: Demonstrate effective teamwork, communication, and collaboration as part of an interdisciplinary care team


What do you bring to the Nest?

- Education & Certification

- High school diploma or equivalent required; bachelor's degree preferred

- Successful completion of an accredited Medical Assistant program

- U.S. work authorization required

- Clinical Experience

- Minimum 1-3 years of relevant clinical experience

- Experience with vital signs, phlebotomy, and assisting in medical procedures

- Electronic medical system (EMR) experience (minimum 1 year), with preference for Athena

- Familiarity with infection control and patient safety protocols

- Experience working with children, teens, and young adults in a healthcare or community- based setting

- Leadership & Training

- Prior leadership experience in a medical office or healthcare setting preferred

- Demonstrated ability to train, mentor, and delegate effectively within a clinical team

- Technology & Administrative Skills

- Proficiency with Microsoft Office Suite (Word, Excel, Outlook)

- Experience using clinical and business tools such as EMRs, Salesforce, and SharePoint

- Strong organizational and time management skills; ability to manage multiple operational workflows

- Ability to advocate for and support families in accessing health and social care services

- Experience in counseling, community outreach, or health navigation is a plus

- Access to reliable transportation and willingness to comply with Nest's Motor Vehicle Safety Policy

What is required (travel & physical requirements)?

- Prolonged periods sitting at a desk and working on a computer

- Must be able to lift up to 15 pounds at times

- Occasional travel to patient homes and Nest offices and supply locations
