Registered Nurse (RN) or Licensed Practical Nurse (LPN) 

  • Salary:  RN $44/hr   LPN up to $32.50/hr
  • Schedule:   Full-time  
  • Shift:  Days 7am - 3pm wth an every other weekend rotation

A little more about our center:

Community Care Center is a large inner-city Skilled Nursing Facility.  We have patients of all ages with multiple levels of care, including long term care and a large behavioral health (psych) population.   

  • We have a new Dialysis program coming in 2025
  • Along with our medical/skilled patients, the most common diagnoses of the rest of our population is schizophrenia, bi-polar and major depressive disorders.
  • The Staff and patients are welcoming and say hello as you walk through the hallways.
  • We are located near W 43 ST & State Street, just off 90/94 and a few blocks from the train station.

The National Association of Rehab Providers & Agencies (NARA) awarded Transitional Care Management their “Impact Award” for 2023 for our Life Skills Programming.  This center is using that very same “Life Skills Program” and changing lives every day.  Ask us about it during your interview – we’d be delighted to share our success stories.  It’s so much more than what your current Psych/Social team is doing now!

Job Description

What you will do in this role: 

As a Registered Nurse (RN) or Licensed Practical Nurse (LPN), you will be joining a team of nursing professionals dedicated to providing excellent clinical care.  Some have been with us for a long time and can mentor others.  You will help patients reach their clinical goals and work side by side with a highly skilled multidisciplinary team.

Qualifications

Floor Nurse:  We will consider a Registered Nurse (RN) or Licensed Practical Nurse (LPN)  for this role.

What we need from you:

  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) in IL
  • Prior floor nurse experience within a skilled nursing facility is helpful
  • Experience with the post-acute patients – specifically the cardiac, post op ortho & the medically complex – is ideal
  • Electronic medical records experience is helpful (Point Click Care or PCC is ideal)

This center is managed by TRANSITIONAL CARE MANAGEMENT.  We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.
