Job Summary

Provides direct palliative care to terminally ill patient and their families while functioning within a medically approved plan of care. The Certified Nurse Assistant will deliver services under the direction of the interdisciplinary team and be supervised by a registered nurse.

Accountability

  • This position reports directly to the Director of CNA Services
  • Works under the supervision of an RN
  • Follows the Plan of Care set out by Interdisciplinary Team

Job Duties and Responsibilities

General:

  1. Demonstrates a thorough understanding of the agency’s mission, philosophy and scope of services.
  2. Demonstrates knowledge and respect of rights and responsibilities by observance of patient/participant: Privacy, confidentiality and safety, including reporting of patient/participant abuse or neglect.
  3. Foster and maintain an effective working relationship between co-workers, patients, families, caregivers, and other professional members of the community.
  4. Demonstrates reliability by: Making every attempt to report to work on time and providing proper notification for absence or tardiness.
  5. Demonstrates ability to problem-solve and make decisions by: Assessing situations and considering all possibilities to resolve the issues then correcting the problems presented in a reasonable amount of time.

  8. Demonstrates effective time management skills by: Coordinating activities to achieve maximum productivity and efficiency, willingness to adjust or revise work schedules as requested, seeking prior authorization for overtime, adhering to the policy for make-up time, completing assigned projects within specified time frames.
  9. Participates in Performance Improvement activities when requested.
  10. Works well independently and within the Interdisciplinary Team structure.
  11. Promotes a team approach through cooperation, mutual respect, flexibility and dependability.
  12. Ensures all regulatory standards for State, Federal and CHAP are met.
  13. Assumes responsibility for on-going personal development and continuing education.
  14. Completes monthly and annual trainings as assigned by the Governing Board, Director of Human Resources and/or Director of Nursing.
  15. Creates opportunities for new referrals and referral relationships when representing the company in the community.
  16. Supports the philosophy, objectives and mission of BeLoved Hospice.
  17. Stays current with all mandatory agency requirements.
  18. Attire to be consistent with the BeLoved Hospice dress code policy.

Additional Functions:

  1. Follows and individualized Plan of Care established by the interdisciplinary team for each patient
  2. Successfully demonstrates the ability to provide the following care(s) to patients:
    1. Bathing (shower, partial, full bed bath)
    2. Oral Care (for an alert patient, an unresponsive patient and a patient with dentures)
    3. Shaving (with disposable and electric razor)
    4. Vital Signs (blood pressure, pulse, temperature and respirations)
    5. Basic Nail Care
    6. Shampoo Patient’s Hair (in shower and in bed)
    7. Use Appropriate Dressing Techniques (for an ambulatory, wheelchair bound, bedbound and unresponsive patients)
    8. Provide Effective Skin Care
    9. Knowledge of Proper Toileting Techniques
    10. Knowledge with Proper Colostomy Care
    11. Knowledge with Urinary Bag Devices
    12. Appropriate Positioning and Body Alignment
    13. Range of Motion Activities
    14. Safe Transfer Techniques
    15. Reporting Change in Patient Condition to RN Case Manager
    16. Notify RN Case Manager when Patient Complains of Pain
    17. Basic Homemaker Services
    18. Light Housekeeping Services
    19. Laundering of Personal Clothing and Linens
    20. Meal Preparation/Feeding
    21. Companionship
  3. Gathers data as assigned by the RN
  4. Immediately reports any changes to the RN Case Manager or Director of Nursing
  5. Reports unusual conditions, observations and pertinent information to the RN Case Manager.
  6. Understands/provides basic principles of palliative care/pain management within the Scope of Practice.
  7. Provides personal care as assigned by the RN, which may consist of but not limited to: Certified Nurse Assistant duties, companionship, light housekeeping, etc.
  8. Comply with all documentation policies/procedures, which include documentation at bedside.
  9. Assumes responsibility for on-going personal development and continuing education.
  10. Creates opportunities for new referrals and referral relationships when representing the company in the community.
  11. Supports the philosophy, objectives and mission of BeLoved Hospice.
  12. Maintains professional competency and licensure.
  13. Other duties as assigned.

Physical/Safety Requirements:

  1. Must have the strength and endurance required for standing, walking, lifting, bending and pushing on a consistent basis greater than 50 lbs.
  2. Utilize appropriate durable medical equipment as needed in a safe manner
  3. Use appropriate body mechanics for CNA duties as identified in the scope of practice
  4. Category 1 for potential exposure to blood/body fluids. (Requires the performance of procedures or other tasks in the work routine that involve an inherent potential for mucous membrane or skin contact with blood, body fluids or tissues, or a potential for spills or splashes of them. Protective measures should be used while engaged in Category 1 tasks with exposure potential.)
  5. Utilizes appropriate PPE as needed.
  6. Follow the designated plan of action in the event of a fire or other emergency.
  7. While performing the duties of this job, the employee is regularly exposed to fumes or airborne particles, work- related stresses, and driving hazards. The individual may be required to drive to meetings with clients or health care professionals. The individual may be required to drive to patient’s homes. The noise level in the work environment is usually minimal.
  8. Utilize appropriate basic office equipment.

Qualifications and Experience

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.  The requirements listed below are representative of the knowledge, skill, and/or ability required to perform this job.

  1. Must be currently certified as a Certified Nurse Aide by the Oregon Board of Nursing.
  2. Prefer one year experience as a CNA in hospice or home health or geriatric care.
  3. Must be able to demonstrate basic CNA qualifications and successfully pass initial direct and indirect supervision competency items prior to direct care with a patient.
  4. Must have reliable transportation and maintain auto insurance.
  5. Successful completion of background check pursuant to ORS 181A.195.
  6. CPR/First Aid Certified
  7. The Home Health Aide may also be referred to as a CNA and will have the ability to perform basic homemaker services as well.
  8. Read, write and understand the English language.
  9. Must be able to handle frequent interruptions, problem solve, and have exceptional follow through skills. 
  10. Must be able to cope with emotionally charged situations encompassing patients, families, caregivers, and team members.
