SFDPH strives to achieve its mission through the work of multiple divisions - the San Francisco Health Network (SFHN), Population Health, Behavioral Health Services, and Administration. The San Francisco Health Network is the City’s only complete system of care and has locations throughout the City, including Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG), Laguna Honda Hospital and Rehabilitation Center (LHH), and over 15 primary care health centers. The Population Health Division (PHD) provides core public health services for the City and County of San Francisco: health protection, health promotion, disease and injury prevention, and disaster preparedness and response. Behavioral Health Services operates in conjunction with SFHN and provides a range of mental health and substance use treatment services.

Job Description

The SFHN Associate Chief Operating Officer, Health at Home & Rehabilitation Services is responsible for two critical services for SFHN: Health at Home and the Integrated Rehabilitation services. Integrated Rehabilitation includes ZSFG and LHH Rehabilitation Services, and provides patient care services in ZSFG Acute Care, Skilled Nursing (4A), Outpatient Services (Ambulatory Care), and at LHH Hospital and Rehabilitation Center.

Essential duties of this position include: 

  • Managing the leadership teams who oversee day-to-day operations.
  • Engaging patients, staff, referring providers, and the hospital administration team with humility and respect.
  • Ensuring successful implementation of organizational and departmental strategic initiatives, using lean leadership tools and methods.
  • Responsibility for all day-to-day operations, adherence to Materials and Supplies and Contract budgets, development and execution of staffing plans, and ensuring Rehabilitation Services aligns procedures and workflows to meet the rehabilitation services needs of patients across the SFHN.
  • Responsibility for ensuring the quality and safe delivery of Health at Home services as required by the Centers for Medicare & Medicaid Services (CMS).
  • Assuming overall responsibility and authority for administrative and supervisory functions and operations of the Health at Home Agency.
  • Coordinating the provision of home care services to reflect the home health care agency philosophy and standards of home health care.
  • Planning, developing, implementing, evaluating, and budget development for the home health care services, programs, and activities.
  • Adopting, reviewing, and approving the bylaws and policies of Health at Home and considering proposed revisions annually.
  • Ensuring that the agency does not refuse service or employment to or in any way discriminate against any person because of race, color, creed, sexual orientation, physical ability, or country of origin.
  • Organizing and directing activities of the program according to its stated purposes and objectives.
  • Conducting Health at Home Professional Advisory board meeting per regulatory guidelines.
  • Developing and implementing an effective budgeting and accounting system.
  • Maintaining written service agreements, including financial arrangements with other approved providers for appropriate services necessary to meet the needs of patients/clients.
  • Ensuring that Health at Home personnel are qualified and regularly evaluated, and that adequate staff education programs are implemented.
  • Ensuring the accuracy of all public information materials including advertisements and brochures, and of all activities.
  • Providing direction and organization for ongoing agency functions.
  • Promoting and maintaining an agency environment that complies with federal, state, and local regulatory agencies
  • The SFHN Associate Chief Operating Officer, Health at Home & Rehabilitation Services may perform other duties as assigned/required.

Qualifications

Required Minimum Qualifications

  1. Education: Possession of a bachelor's degree from an accredited college or university, AND
     
  2. Experience**: Five (5) years of verifiable experience managing acute, post-acute, ambulatory, rehabilitation, and/or home health services all of which must include supervising professionals, AND
     
  3. LICENSE**: Possession of a valid, unrestricted license issued by the State of California as a
    1. Registered Nurse (RN),
    2. Registered Physical Therapist (RPT),
    3. Registered Occupational Therapist (OTR),
    4. Speech Language Pathologist (SLP),
      OR
    5. Registered Respiratory Therapist (RRT).

*EDUCATION SUBSTITUTION: Additional professional experience as described above may substitute for the required degree on a year-for-year basis up to a maximum of two (2) years. One year is equivalent to thirty (30) semester and forty-five (45) quarter units.

One-year fulltime employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40-hour work week).
