Position Overview
GreenLife Healthcare Staffing is currently hiring a Licensed Master Social Worker (LMSW) to serve as a Nursing Home Surveyor / Complaint Investigator in Central Islip, NY. This role involves conducting site visits, regulatory compliance surveys, and complaint investigations in long-term care facilities.
Responsibilities
- Conduct surveys and complaint investigations, including allegations of abuse or neglect
- Draft Statements of Deficiencies (SOD) using Principles of Documentation
- Assess compliance with Plans of Correction
- Enter data into Federal and State reporting systems
- Assist with Immediate Jeopardy monitoring and state oversight
- Attend meetings and testify in administrative hearings if required
- Follow DOH and IPRO policies and schedules
- Travel locally to assigned sites as scheduled
Schedule
- Full-Time
- Monday to Friday
- Flexible Hours: 7:00 AM – 7:00 PM
Requirements
- Active NYS LMSW license (required)
- Minimum 2 years of clinical experience in a medical setting (nursing home preferred)
- Preferred: 2 years’ experience in nursing home surveillance
- SMQT Certification preferred or must be willing to obtain within 12 months
- Must be able to travel to assigned onsite facilities
Compensation & Benefits
- $39/hour
