Seeking Home Health Licensed Vocational Nurse Full-Time in the Austin, TX and surrounding areas.

Shift/Schedule: Mon-Fri 8-5 p.m. with 1/On call weekend per month for full time and we also have PRN available!

Responsibilities

  • Skilled Visits
  • Assists in development, review and revision of the plan of care.
  • Provides quality patient care set forth by the RN Case Manager's Care plan
  • Observes, records and reports reaction to treatment and any changes in client’s condition to appropriate personnel and/or physician.
  • Provides ongoing instruction and supervision of client, family or other health team personnel in the plan of treatment.

Requirements:

  • Geriatric Home Health Experience
  • Texas Nursing License
  • Strong organizational and self-management skills
  • Strong and compassionate customer service skills
  • Valid driver’s license
