What does success look like as our new Home Care Supervisor?
  • Building Excellence: Collaborate with cross-functional myPlace Health team members to design and implement a high-quality Care At Home Model, ensuring participants’ care needs are consistently met.
  • Team Leadership: Oversee and manage a team of Home Care Coordinators, Schedulers, and Home Care Attendants who deliver activities of daily living (ADL) services such as bathing, grooming, dressing, toileting, and transferring in participants' homes.
  • Recruitment and Training: Interview, hire, onboard, train, and provide feedback to Home Care team members to ensure their skills, knowledge, and credentials meet myPlace Health standards.
  • Vendor Oversight: Build and maintain relationships with contracted vendors, providing feedback on caregiver performance and participant interactions to ensure alignment with our values and care standards.
  • Quality Assurance: Regularly audit Home Care documentation for accuracy and timeliness, offering training and coaching as needed to drive continuous improvement.
  • Empowering Care Teams: Partner with interdisciplinary team members to ensure Home Care staff are fully trained and equipped to deliver care as outlined in individualized participant care plans.
  • Process Improvement: Collaborate with functional team leads to develop and refine policies and processes in alignment with myPlace Health standards, PACE guidelines, and CMS regulations.
  • Contribute to Organizational Growth: Embrace additional opportunities and responsibilities as needed, actively participating in the continued growth and success of the organization.


What does an ideal Home Care Supervisor candidate look like?
  • Education: High School Diploma or equivalent (a Bachelor’s Degree is a plus!).
  • Licensure: Active LVN license in good standing.
  • Experience: 1-2 years as a Home Care Coordinator or in a similar role; At least 1 year managing a team of Home Care Attendants, particularly working with frail or elderly populations.
  • Supervisory experience: 2+ years of supervisory experience with a proven track record of mentoring and developing team members.
  • Transportation: Reliable means of getting around to support home visits and team needs.
  • Has a Passion for Service: Dedicated to serving high-risk seniors and frail older adults with dignity, respect, and compassion.
  • Brings Leadership Experience: Proven leadership skills, ideally in home care or healthcare settings.
  • Is Organized and Personable: Strong organizational abilities and excellent interpersonal skills to juggle multiple priorities effectively.
  • Knows Healthcare Programs: Familiarity with PACE or similar healthcare programs is a plus!
  • Can Mentor and Empower: Demonstrated ability to train, mentor, and support team members to deliver outstanding care.
  • Understands Regulations: Familiar with regulatory requirements like CMS and PACE (a bonus if you already have experience).


$90,000 - $105,000 a year

Base pay is based on several factors including but not limited to education, relevant work and industry experience, certifications, and location of the role. Onsite roles include appropriate geographic adjustments, while remote roles are typically priced off national pay data.
