The advanced practice provider (Nurse Practitioner) within the ambulatory setting works in collaboration with and is an integral member of a multidisciplinary health care team. The Nurse Practitioner provides comprehensive care in the Jefferson Occupational Health Network ambulatory practice. Assessment amp; Management Perform complete history amp; physicals and document in electronic medical record. Order and collect data using appropriate assessment techniques, supporting diagnostic information and diagnostic procedures where indicated. Collaborate on health surveillance programs to identify potential workplace health hazards including health screenings and vaccine administration. Collaborate and monitor infectious disease exposure testing, follow-up, and education. Provide direct medical care and management of work-related injuries (including bloodborne exposures) as well as provide education on workplace injury and illness prevention. Provide episodic, primary care to student population. Serve as the first responder in the event of employee status changes in situations requiring resuscitative interventions. Act as a clinical resource for registered nurses and medical assistants. Diagnosis Collaborate with interdisciplinary health care team to manage diagnostic tests through ordering, interpretation, performance, and supervision. Formulate differential diagnoses by priority. Diagnose complications and orders appropriate interventions. Formulates Plan of Care Identify expected outcomes from diagnosis; formulate and documents a plan of care to address work related injury. Utilize evidenced based practice guidelines and protocols in an individualized, dynamic plan of care. Implement and modify plan of care. Prescribe diagnostic strategies and therapeutic interventions. Documentation Document key components of employee39;s progress via note where applicable. (hamp;p, progress notes, plan of care, problem list, procedure notes, discharge summaries, in electronic medical record) Communication and Collaboration Maintain ongoing communication and collaboration with interdisciplinary health care team. Facilitate and communicate with employees, volunteers, and students to promote continuity of care across the inpatient and outpatient continuum of care. Professional Practice Demonstrate Professional Practice behaviors. Seeks opportunities for active engagement in analysis of evidenced based practice. Actively participate in Occupational continuing education and office clinical meetings and huddles. Actively maintain CEUs, and membership in a professional organization. Master of Science in Nursing (MSN) or a Doctor of Nursing Practice (DNP) National Board Certification as an Adult/Family Care Nurse Practitioner 3 -5 years of Primary; Workerrsquo;s Comp Experience Preferred BLS certification Covid Vaccination is a requirement for employment at Jefferson for employees working at Jeffersonrsquo;s clinical entities or at the University.
