Dignity Health’s Pacific Central Coast Health Centers (PHC) is a non-profit Community clinic organization comprised of more than 40 health centers located primarily from Ventura to Templeton spanning the coast. Our experienced physicians and advanced practice providers offer a wide range of primary and specialty care services. PHC is a part of Dignity Health Central Coast’s award-winning network of outstanding hospitals imaging centers laboratories and post-acute services.

Responsibilities

The Medical Safe Haven Patient Advocate provides support services for the Medical Safe Haven, supporting continuum of care for patients who are victims and survivors of human trafficking. This position will serve as a liaison between patients, healthcare providers, and external agencies. In addition to providing patient support, the Medical Safe Haven Patient Advocate will work with MSH program staff and partner agenciesto increase awareness and provide a bridge to referrals and improve access to comprehensive care. This includes community outreach and engagement such as representing the Medical Safe Haven program at conferences, healthcare MSH curriculum training, community events, and partner meetings.

Qualifications

  • AA or Bachelor’s degree preferred or 3+ years experience working in social services, or organization serving at-risk populations.
  • Bachelor’s Degree in Public Health, Social Work or other related field of study that demonstrates a knowledge of vulnerable and at-risk patient populations, or 3+ plus years’ experience in working with vulnerable and at-risk patient populations.
  • Comprehensive knowledge of project support
