Join the Cleveland Clinic team where you will work alongside passionate caregivers and provide patient-first healthcare. Here, you will receive endless support and appreciation while building a rewarding career with one of the most respected healthcare organizations in the world.     

The East1, East2, West and South Home Care Teams cover specific geographical areas where each caregiver is assigned to a smaller service area within the team. As part of the East 2 team, you will facilitate high quality nursing care to patients in the comfort of their own homes. You will provide assessments, complex nursing treatments and education to help them avoid readmissions. This position offers a unique opportunity to meet patients in a familiar setting where they feel safe with being open, vulnerable and receptive.  

A caregiver in this position works three 12-hour shifts from 10:00AM – 10:30PM, with no on-call, weekend or holiday requirements. 

A caregiver who excels in this role will:  

  • Provide direct nursing care in accordance with established policies, procedures and protocols of the agency, taking visits on the uncovered list, late admissions and other patient visits as needed.  
  • Participate in and support quality improvement projects. 
  • Participate in departments and committees as assigned. 
  • Supervise starts of care, resumptions of care and routine visits as requested. 
  • Perform accurate and complete admission assessments and reassessments within a specified time frame. 
  • Assess patient for safety risks, abuse and neglect. 
  • Serve as a primary coordinator of all disciplines for well-coordinated patient care. 
  • Assess and document patients' pain using appropriate pain scale and manage patients' pain then. 
  • Prioritize and document patient problems based on assessed needs and problems in collaboration with patients, family and healthcare team. 
  • Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions. 
  • Escalate change in patient condition to patient’s physician as appropriate.  

Minimum qualifications for the ideal future caregiver include:  

  • Graduate from an accredited School of Professional Nursing 
  • Current state licensure as a Registered Nurse (RN) 
  • One year of current clinical experience as an RN 
  • One year of experience in an acute medical/surgical or homecare setting 
  • Valid Driver’s License (VDL) and proof of $100,000/$300,000 automobile insurance coverage 

Preferred qualifications for the ideal future caregiver include: 

  • Bachelor’s of Science in Nursing (BSN)

Physical Requirements:

  • Requires full range of motion, manual and finger dexterity and eye-hand coordination.
  • Requires standing and walking for extensive periods of time.
  • Requires corrected hearing and vision to normal range.
  • Requires some exposure to communicable disease or bodily fluids.
  • Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
  • Physical Demand requirements are in excess of those for Light Work.
  • Must be ambulatory and able to drive to patients' homes, able to climb stairs, pull, push, and facilitate movement of the patient.
  • Must be able to read, write and communicate with patients.

Personal Protective Equipment:

  • Follows standard precautions using personal protective equipment as required.
