RN Supervisor - Home Care

FellowshipLIFE fosters a work environment grounded in integrity, inclusivity, and a genuine sense of community. We are currently seeking a Home Care Visiting RN Manager to support our non-medical Home Care program across the greater Freehold and Whiting areas—serving vibrant neighborhoods throughout Monmouth and Ocean Counties, and other nearby communities.

In this vital role, you'll advocate for the well-being of our clients by leading and supporting CHHA’s in the field, ensuring quality, personalized care, and maintaining best practices and compliance with all standards. If you're a compassionate RN with a passion for senior care and a drive to make a meaningful impact, we’d love to welcome you to our team.



Key Responsibilities:
  • Ensure compliance with Board of Nursing Best Practices and CAHC Accreditation Standards
  • Communicate openly with clients, families, and team members
  • Supervise and support CHHAs 
  • Formulate initial admission and care plan  
  • Conduct necessary follow-up visits and evaluate care effectiveness
  • Oversee onboarding of CHHAs and collaborate with leadership team
  • Provide in-services, skills assessments and ensure all required documentation is completed accurately


Requirements:
  • Valid New Jersey RN License & BLS Certification
  • Active NJ Driver’s License and reliable vehicle with insurance
  • 1+ year of experience in healthcare (home health, hospital, or LTC preferred)
  • Participate in on-call rotation
  • Self-starter with excellent communication, critical thinking, and organizational skills
  • Proficiency with electronic medical records and related software
