Position Summary:

The Patient Navigator will provide case management and social work services to PPLM patients

statewide. They will be assigned cases by the Manager of Patient Services and work to address patients'

unmet bio-psycho-social needs. The Patient Navigator will collaborate with multidisciplinary staff at the

health centers and they will assess patients’ needs and offer supportive counseling and referrals to

medical services and community resources. The Patient Navigator will escalate high-risk cases to the

Manager of Patient Services as needed.


Responsibilities:
  • Complete formal assessments of patients in the electronic health record and document ongoing progress in charts.
  • Assess patients’ bio-psycho-social needs and offer short-term supportive counseling and referrals to community resources as needed.
  • Collaborate with multidisciplinary team at PPLM to address patients’ needs.
  • Update referral lists as needed to ensure accurate information is being shared with patients.
  • Provide crisis intervention and support to high-risk patients.
  • Collaborate with Southeastern MA Community Outreach Specialist to offer specialized support in the Southeastern area of the State.
  • Represent PPLM at various meetings including DPH meetings.
  • Offer coverage for abortion Access Program Patient Navigators as needed.
  • Participate in weekly supervision with Manager of Patient Services.
  • Serve as a Certified Application Counselor under PPLM’s designation by the Executive Office of Health and Human Services and the Commonwealth Health Insurance Connector Authority.
  • Participation in PPLM’s efforts to achieve goals for productivity.
  • Other tasks and responsibilities as needed.


Qualifications
  • 1-3 years of work experience working in diverse social service communities.
  • BSW or MSW preferred, relevant experience highly valued.
  • Bilingual Spanish/English preferred.
  • A flexible schedule is a must, including evenings and weekends as needed.
  • Consistent access to a vehicle, valid driver’s license, and willingness to occasionally drive to all Health Centers in MA.
  • Computer experience, especially Microsoft Office Suite, required.
  • Experience in crisis intervention, case management, counseling, and/or advocacy preferred.
  • Experience working in a health care setting preferred.
  • Familiarity with Massachusetts health insurance programs preferred.
  • Demonstrated ability to work with people of different races, ethnicities, sexual and gender identities and expressions, ages, abilities and experiences.
  • Commitment to addressing the impact of racism and other oppressions on sexual health.
  • A commitment to PPLM’s mission and values.


$25.50 - $25.50 an hour
This position is part of the Collective Bargaining Agreement.

Roles that are denoted as Hybrid require 1 day per week in the office unless the role is denoted as onsite, which requires working onsite full time or 5 days per week.
