Position Summary

Baptist Health Princeton Hospital

A recognized leader in aortic valve surgery, COPD, heart attack, heart failure and stroke, we provide fast, lifesaving response times for patients with chest pain or stroke symptoms. Our full range of services includes comprehensive emergency room care; cardiovascular care, including structural heart and valve care; orthopedics, including rehabilitation; a comprehensive sleep center; a comprehensive bariatrics center, providing surgical and nonsurgical weight-loss procedures; and women’s care for all stages of life.

We have achieved Primary Stroke Center certification from The Joint Commission and accreditation from the American College of Surgeons Commission on Cancer and from the American College of Surgeons Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program (MBSAQIP). Position Summary

Interacts directly with adult and adolescent patient/family to address issues/concerns of a clinical nature (medical necessity, mentalwellness, linking patient care, etc.) as appropriate to facilitate efficient movement through the continuum of care

Responsibilities

Essential Functions• Initially and concurrently assesses all adult and adolescent clients within assigned population to include but not limited to:diagnosis/medical history; current treatments/therapies; age; payment source; criteria compliance; resources; support systems; anticipated needs; appropriate level of-service;special/personal needs; and any other relevant information.• Obtains referrals from Behavioral Health specialty medical and mental health providers and other referral sources to assess adult and adolescent client needs.• Acts as an advocate for patient’s health care needs.• Provides outreach and education at health fairs and other community events• Facilitates family meetings to discuss lab results, provider recommendations, and plan of care with client’s care team.• Maintains records and documentation of work performed in an organized and easily retrievable fashion• Maintains positive relationships with supervisor, peers, collaborative team, families, and community contacts.• Maintains reasonably regular, punctual attendance consistent with Baptist Health policies, the ADA, FMLA and other federal, state and local standards.• Maintains compliance with all Baptist Health policies and procedures.

Other Related Functions• Maintains confidentiality of data and information in compliance with HIPPA regulation and Baptist Health policies.• Completes orientation and department specific competencies.

Qualifications

Education/TrainingBachelor’s degree in psychology, social work, or healthcare related field

 

Licensure/CertificationValid Alabama driver’s license with no more than six (6) points, at least 21 years of age, and must be insurable under the BaptistHealth insurance carrier’s requirements and standards

 

ExperienceOne (1) year experience in case management or related patient care support role working in community agency, out-patient facility, or hospital setting.
