Responsibilities

A Lead Community Health Worker (CHW) serves as a bridge between community members and healthcare and social service systems, advocating for their needs and promoting health equity. They lead and mentor a team of CHWs, providing guidance, training, and support to ensure effective outreach, education, and navigation services are delivered. In addition to direct client interaction, the Lead CHW is responsible for program development, data collection, reporting, and fostering strong relationships with community partners to address social determinants of health and improve overall community well-being. They play a crucial role in empowering individuals to take control of their health and accessing the resources they need to thrive.

 
  • Prepares external and internal communication and related material for department asrequested from verbal direction or from knowledge of systems policy and/or procedure
  • Establishes and accurately maintains files, database (s), records and ongoing basis for the department
  • May coordinate and lead evidence based community health programs
  • Partner with other Community Health Workers and community health organizations in the community
  • Educate patients on Pathways to Healthier Living Program, offer enrollment, and maintain a caseload of program participants. Conducts home visits in accordance with PCHI standards.
  • Serve as lead program contact when supervisor is unavailable. Assist in completing and submitting program deliverables.

Qualifications

Minimum: 

  • High School Diploma/GED
  • Valid Arizona Drivers License
  • Valid Arizona Car Insurance
  • Computer skills, MS and Google suites
  • Experience working in the community, Community Health Worker experience

Preferred: 

  • Bachelors Degree
  • Completed CHW or Promotora Training
  • Teaching
  • Experience in grants,research, community health planning,strategic thinking, and organizational skills
