Job Description

We are seeking an experienced Clinical Pharmacist II to join our healthcare team in Bel Air, MD. In this role, you will provide comprehensive clinical pharmacy services, medication therapy management, and patient care while ensuring compliance with all regulatory requirements and safety standards.

  • Evaluate and optimize medication therapy through clinical interventions and monitoring
  • Provide comprehensive medication therapy management services to patients
  • Collaborate with healthcare providers to develop and implement patient care plans
  • Review and assess drug utilization patterns for safety and effectiveness
  • Conduct medication reconciliation and therapeutic drug monitoring
  • Provide drug information and clinical consultations to healthcare professionals
  • Document clinical interventions and patient care activities in electronic health records
  • Participate in multidisciplinary rounds and patient care conferences
  • Ensure compliance with pharmacy regulations and safety protocols
  • Mentor pharmacy residents and students when applicable

Qualifications

  • Doctor of Pharmacy (Pharm.D.) degree from an accredited institution
  • Current and valid Maryland state pharmacist license or eligibility to obtain one
  • 3-5 years of clinical pharmacy experience in a hospital or clinical setting
  • Proven expertise in medication therapy management and clinical pharmacy practices
  • Strong knowledge of clinical pharmacology and drug therapy
  • Proficiency in electronic health record systems and drug information databases
  • Excellent analytical and problem-solving abilities
  • Strong documentation and clinical writing skills
  • Demonstrated ability to work collaboratively in a multidisciplinary healthcare team
  • Experience in mentoring pharmacy residents and students preferred
  • Comprehensive understanding of pharmacy regulations and healthcare compliance
