Position Purpose: Supports care management activities and the teams assigned to members to ensure services are delivered by the healthcare providers and partners and continuity of care/member satisfaction is achieved. Interacts with members by performing member outreach telephonically or through home-visits and documents the plan for care/services of activities.

In this position, you’ll be directly interacting with members, either over the phone or during home visits, to discuss their care plans and ensure they navigate their healthcare effectively. You’ll coordinate care activities, linking members with healthcare providers and community resources to facilitate continuity of care. With emphasis on member satisfaction, your role will include documenting care plans, performing assessments, and providing education about available benefits. Plus, you will serve as a crucial support system for any inquiries or issues that arise regarding their care. Ensuring compliance with regulatory guidelines and best practices will also be part of your duties.
