Homeless Outreach Mental Health Practitioner

***The expected salary range for this position is based on several factors including market, funding, candidates experience level, skills, training, and related knowledge. Our ranges are developed and published as good faith estimates for the position.***

Starting Salary Range: $59,773 - $62,987 + Office Benefit Package (Parking/Remote Expenses): $180/Month subject to change

The Homeless Outreach Mental Health Practitioner will provide professional, quality program services to the persons served by the PATH Homeless Services Team. The Homeless Outreach Mental Health Practitioner will work with members of the team to assist persons served in obtaining and maintaining an optimum quality of life consistent with client needs and strengths of the homeless population. Emphasis is placed on working with persons who are unsheltered and have a mental health and/or co-occurring diagnosis; directly providing the services necessary to assist individuals in achieving their goals and obtain housing.

Hours: Monday - Friday 8 am - 4 pm or 9 am - 5 pm, 40 hours per week

**This position works onsite at the RADIAS Health downtown Saint Paul office**

Duties and Responsibilities:

  • Provide PATH services (outreach, screening, case-management, re-engagement) to PATH eligible/enrolled clients, assume primary responsibilities for developing, writing, implementing, evaluating, and revising recovery/housing goals and plans in collaboration with the PATH client.
  • Maintain an active caseload of 15-20 clients.
  • Work within a multidisciplinary team consisting of an Advanced Nurse Practitioner, Mental Health Professional/Practitioners, Certified Peer Specialist.
  • Identify candidates for the Section 811 and PHOP subsidy programs, complete the application process and start referral process to HSS. Once a client is approved, engage in coordinated outreach efforts with RADIAS Health Section 811 team members and/or the RADIAS Health Housing Navigation Team to the client to begin the transition of services.
  • Target outreach efforts to individuals experiencing homelessness with high acuity and complex needs
  • Ensure voices of individuals with lived experience of homelessness is integrated into PATH program planning and outreach efforts by conducting Satisfaction Surveys to PATH enrolled clients.
  • Provide daily community outreach and engage with people living in places not meant for human habitation as well as in shelter settings and community drop-in locations.
  • Engage in coordinated outreach efforts with RADIAS Health outreach staff and other community outreach providers; these may occur during early morning hours or in the evening.
  • Coordinate cases with other county or community agencies including Intake, Mental Health Case Management, Adult Protection, Income Maintenance, Crisis Center, and community hospitals.
  • Develop appropriate care plans with enrolled clients.
  • Initial assessment to determine presence of mental illness, chemical dependency, or behavioral problems.
  • Monitor clients who may decline active services who represent a potential danger to self or others to ensure referral for emergency psychiatric or other crisis intervention services as needed.
  • Coordinate service provisions with other service providers to ensure smooth, prompt, consistent and quality delivery of services to clients.
  • Refer urgent cases where there is risk of harm to self or others to the appropriate hospital emergency room and community crisis teams.
  • Provide information and referral, intervention strategies and assistance in deescalating the crisis, if there is no imminent risk of harm to self or others.
  • Assess the need for hospitalization, crisis residence, or Detox services.
  • Provide Client Advocacy and Information and Referral to clients and agencies to ensure protection of civil rights and welfare of clients.
  • Refer cases to Adult Protection and Child Protection where there is evidence of neglect and/or abuse of Vulnerable Adults and/or children.
  • Assist clients in obtaining needed legal and/or advocacy services.
  • File all legally required reports with Vulnerable Adults, the Ombudsman and Child Protection.
  • Provide education to clients regarding the symptoms, course, and treatment of their illness.
  • Must be able to transport clients in the community.
  • Complete all paperwork, reports, and charting contemporaneously and in an organized manner.
  • Participate actively in on-going professional growth and development; maintain appropriate professional behavior and participate in appropriate supervision.
  • Allow for flexibility in hours worked.

  • Bachelor’s degree in behavioral sciences or related fields
  • Experience providing outreach services to persons experiencing homelessness and knowledge of services/resources in Ramsey County.
