​Job Description: 

We are seeking a compassionate and experienced Family Medicine Nurse Practitioner (FNP) or Physician Assistant to join our dynamic team at Foundation Medical Partners / Southern NH Health. In this role, you will play a vital role in providing comprehensive care to a diverse patient population in Pepperell, MA.

  • Collegial practice with solid APP support
  • W2, employed position with Foundation Medical Partners
  • Not-for-Profit, 501(c) Organization
  • 36 Patient Hours/4 admin hours per week
  • Clinical Affiliate of Massachusetts General Hospital
  • Convenient Southern NH/MA Border Location, minutes to the mountains or ocean

Qualifications:

  • Master’s degree in nursing
  • 2+ years of Family Medicine experience
  • Strong clinical skills, including assessment, diagnosis, and treatment planning.
  • Excellent communication, collaboration, and interpersonal skills.
  • Proficiency with EPIC EMR preferred
  • Ability to work independently and as part of a team.

Work Shift: 

8am-5pm
