Mae is looking for a Care Coordinator who is passionate about maternal health equity to support Medicaid payer pilots and member level engagement to address the growing disparity in Black maternal health outcomes.
As a care coordinator, you will be responsible for supporting and navigating care for members of key payer contracts across a variety of Medicaid landscapes.
The care coordinator will be responsible for maintaining positive relationships with internal and external parties, being the liaison between the member, the Mae team and partner care management teams, and ensuring all documentation has been completed.

Whether you’re conducting needs assessments that look into various aspects of a member’s life or utilizing your motivational skills to maximize their involvement in healthcare decisions, every day will be an opportunity to make a significant impact. This is a hands-on role where you will manage care plans, facilitate communication, and identify high-risk factors, all aimed at ensuring that no gap in care goes unnoticed. If you bring 3-5 years of experience in social work, nursing, or healthcare, along with strong organizational skills and a knack for relationship management, you might be the perfect fit for this position. With a flexible work schedule and an hourly rate of $32 - $38 based on your experience, this contract role allows you to create positive change in the lives of mothers and their families while also offering you the flexibility to manage other commitments.
