FT Nurse Practitioner

As a Full-Time Nurse Practitioner at Pine Park Health, you will provide onsite primary care services to older adults living in Senior Living Communities. Your responsibilities include developing and executing individualized care plans, conducting monthly Chronic Care Management visits, and collaborating closely with facility staff to enhance patient outcomes. You will also review and sign off on clinical documentation and address issues affecting daily practice.

To qualify for the Nurse Practitioner position at Pine Park Health, you must hold a Nurse Practitioner certification with a passion for geriatric care. While prior experience in geriatric healthcare is preferred, adaptability and strong communication skills are essential. You should also be tech-savvy and open to providing clinical services across multiple senior communities in Fremont, California.
