Hospital at Home (HaH) Nurse/Command Center Nurse:

We are seeking a full-time Registered Nurse for the Hospital at Home (HaH) Program at Fargo Sanford. In this innovative role, an RN would have the opportunity to care for a wide range of inpatient medical-surgical patients in the patient’s home setting and act as a command center nurse, virtually connecting with the patient from our hub located at SMCF. If you are an RN who enjoys variety and pioneering new care platforms this may be the position for you!

Position Highlights:

  • Career Advancement: Opportunity to toggle between command center role and inpatient nurse in the home setting
  • Clinical Team: Work with a small team that is focused on launching this new innovative platform to meet the needs of our growing community.
  • Facilities: Enjoy the variety of working in our remote hub and in a patient’s home setting. Car provided by Sanford for use during working hours.
  • Development: Grow and hone your inpatient nursing skills by applying them differently than we ever have with this exciting new initiative.

Command Center Nurse:

  • 12 hr day and/or night shifts, rotating weekends and holidays
  • Responsible for triaging patient calls
  • Activate HaH nurse to the patient home
  • Coordinate remote visits for the interdisciplinary team
  • Work in our remote hub located at SMCF
  • Utilize Medical Surgical nursing skills to care for a select group of inpatient status patients in the patient’s home setting utilizing remote technology and innovative tools to accomplish your nursing care.

What you will need:

  • ASN or BSN Degree in Nursing
  • Active Registered Nurse License in North Dakota & Minnesota
  • Active Driver’s License

Responsibilities

  • Plans and coordinates patient care, assessment, education, triage and various other nursing interventions
  • Collaborates with colleagues, including physicians, to plan, implement and evaluate care
  • Empathetically cares for patients during all stages of preventive services, health maintenance, diagnosis, treatment and follow-up along the continuum of care
  • Demonstrates competency and uses comprehensive nursing expertise

Qualifications

Graduate from an accredited nursing program required, including, but not limited to, American Association of Colleges of Nursing (AACN), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Bachelor’s Degree in nursing preferred.

Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications.
